Support Planning

 

Aims

A one-day training session which aims to give participants the skills they need to draw up clear individual support plans in consultation with service user to meet individual needs.

Learning Outcomes

By the end of this course participants should be able to:

  • Identify the 6 C’s of care and how this links to support plans
  • Describe the reason for and principles of support planning
  • Describe the support planning process including: assessment, goal setting, action planning, implementation, monitoring and review
  • Carry out an assessment of need
  • Set a goal and break it into steps to create an action plan
  • Identify the process for monitoring and reviewing goals
  • Identify how risk assessments link to support planning

 

Training Methods

The day contains a variety of pair and group work, general discussions, case studies and a limited amount of lecturing.
